In accordance with Texas Health and Safety Code §181.105, the following information is provided regarding access to health care records, the applicable licensing authority, and the consumer complaint process.
REQUESTING YOUR HEALTH CARE RECORDS
Current clients may submit a written records request through secure messaging in the Simple Practice Client Portal.
Former clients may email hello@wildfellpsychotherapy.com with “Records Request” in the subject line to receive instructions for submitting the request securely. Please do not include sensitive clinical information in an email.
Requests should identify the client’s full name, date of birth, the records or dates of service requested, and current contact information. Identity verification and a written authorization may be required before records are released. Requests will be processed in accordance with applicable Texas and federal law.
